As the Hospice Aide, you are a paraprofessional and valued member of the interdisciplinary care team. You will work under RN supervision and focus on quality, continuity, and safe patient care. In this role you will be responsible for timely and accurate documentation of care.

JOB RESPONSIBILITIES

  • You will provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment
  • You will offer positive, respectful, and confidential patient/family communication
  • You will be responsible for taking and recording vital signs, and assisting with self-administered medications
  • You will plan and prepare nutritious meals, including shopping and feeding the patient as needed
  • You will assess and report patient’s condition and changes to the RN Case Manager
  • You will assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities

SKILLS REQUIRED

  • Excellent interpersonal skills such as communication, empathy, and compassion
  • Demonstrated tact and patience
  • Time management and organization skills
  • High integrity and a commitment to HIPAA privacy standards
  • Comfortable providing specialized care to the terminally ill

QUALIFICATIONS

  • At least 18 years of age
  • Active Certified Nursing Assistant (CNA) designation current with state-specific requirements
  • Licensed and insured driver with automobile in good working order
