Job Description and Hours
Provide assistance and support to eligible caregivers to ensure they receive opportunities for respite and other supportive services to reduce the stress associated with caregiving. Gain understanding of each eligible caregiver’s strengths and needs through providing case management services including comprehensive and continuous assessment, participant-centered care planning, and ongoing monitoring, consultation and support.
ESSENTIAL JOB FUNCTIONS:

Reports to work as scheduled on a regular and reliable basis.

Successfully complete all required pre-service orientation and training.

Must insure confidentiality of all client and care receiver information and act in accordance with HIPAA regulations.

Ability to understand and comply with various laws, rules, regulations, policies and guidelines as they pertain to both ENOA and ENHSA.

Ability to communicate clearly, both orally and in writing and be able to establish effective working relationships with many different people, ranging from directors, coordinators, professionals, community representatives, support staff and the general public.

Be a strong team player with positive attitude toward working with staff and with clients and their formal and informal support systems.

Must have good computer skills with general knowledge of Microsoft Office software and have the ability to learn other computer systems.

Conduct a comprehensive face to face assessment of the elderly in their home environment. Write a comprehensive, workable care plan in conjunction with written assessment. The care plan must address the service needs which were discovered in the assessment process. Provide ongoing monitoring as needed to ensure quality delivery of services.

Link clients to those services that impact on their situation and presenting problem. Provide follow-up to assure that the client and the services have interacted.

Provide information, support and encouragement to assist the client and family or significant others in meeting related health and self-care needs.

Inform the Agency staff and all medical professionals involved in the client care of any changes in condition and needs.

Advocate for the rights and dignity of people in need of help, and facilitate a community awareness of those needs.

Ability to professionally collect, document, interpret and utilize program related information using interviewing and listening techniques, program/environment evaluation.

Basic knowledge of geriatrics and knowledge of the client’s needs on an economic, social, medical and community level. Ability to use clinical social work skills in advocating for the client in providing access to community services.

Ability to meet and work with the public. Demonstrate communication and leadership with healthcare team members, community agencies and resources to facilitate maximum client outcomes and establish positive working relationship with them and ENOA.

Must have a valid driver’s license, reliable vehicle and state required car insurance.

OTHER JOB DUTIES:

Participate and potentially develop in-service training programs to staff and others in the community.

Act as an active participant of the Care Management Team and service in activities deemed necessary to the functioning of the Choices Division and ENOA.

Other appropriate duties as assigned by the Choices Division Director.

EDUCATION TRAINING AND EXPERIENCE:

1. State of Nebraska Licensed Nurse, Baccalaureate degree in the field of social work, sociology, gerontology, or applicable human services plus minimum of two years’ experience in gerontology, long-term care, community health, public health nursing or home health nursing preferred.

SKILLS AND ABILITIES:

Must type proficiently.

Knowledge of the geographies, service programs, and resources of community to be served.

Ability to be mobile within that geographic area to effectively meet service demands.

Ability to use discretion in independently organizing and prioritizing work flow, ensuring timelines and professionalism in all functions.

Must have good computer skills with general knowledge of Microsoft office and have the ability to learn other computer systems. Must have the ability to use office equipment; such as fax, scanner, copier, printer, and other equipment as needed. Must be able to drive year around in a variety of road conditions.
